How do I complete my Stipend Survey?
Before opening your secure Stipend Survey, collect the following documents:
- Completed W-9 form: The IRS requires that TNTP collect a completed W-9 form from all participants who wish to receive the stipend. Blank W-9 forms can be found here.
- Bank account and routing information: This information is needed to deposit your stipend into the correct account.
- Voided check or snippet of your bank account information: Submit a blank, voided check. If you do not have checks, you can get a snapshot of your banking information by taking a screenshot of your bank account number and routing number through your bank's online portal.
CRIMSI will not accept these details via email; they must be submitted through the secure survey.
