How and when will W-9 and Banking information be collected?
W-9 and Banking Information will be collected from qualifying participants before each stipend disbursement. To collect your W-9 and Banking Information, we email participants a secure Qualtrics Stipend Survey. Qualtrics is a secure tool for collecting your W-9 and banking information and is also FedRAMP certified.
Important Note: Once you access your Stipend Survey, you will have 15 minutes to complete the form, or the portal will close and require you to restart your submission. Forms will be sent for each disbursement.
The survey does not store incomplete responses, so please have the following documents ready:
- Completed W-9 form: The IRS requires that TNTP collect a completed W-9 form from all participants who wish to receive the stipend. Blank W-9 forms can be found here.
- Bank account and routing information: This information is needed to deposit your stipend into the correct account.
- Voided check or snippet of your bank account information: Submit a blank, voided check. If you do not have checks, you can get a snapshot of your banking information by taking a screenshot of your bank account number and routing number through your bank's online portal.
Please do not submit Social Security numbers or banking information via email, as it will not be accepted.
