What if a participant cannot attend their scheduled CoP? How do participants submit a formal CoP change request?

Given CoP assignments were based on availability shared by district and campus leaders, CRIMSI's policy for rescheduling a participant's CoP assignment involves submitting a formal request. 

Requests will be reviewed in conjunction with district and campus preferences for CoPs, so we encourage participants to share the specific rationale for the need to change a CoP. If CRIMSI cannot accommodate a change request, the impacted participants will need to attend make-up CoP sessions for credit.

Participants may complete the following form to request a formal CoP change by September 13. CRIMSI will review change requests in the order received. Please note that participants needing a CoP change for both piloted products should submit this form once per product.
